Preparing For Your First Appointment
At your first appointment, a nurse will provide you with information on the Affordable IVF model of care along with a break down of costs. This appointment lasts approximately 45 minutes. In preparation for your first appointment, please print and complete a Fertility Assessment Questionnaire. If you have had fertility treatment at another clinic, please contact the clinic and get a copy of all your results and cycles and bring these with you to your first appointment. Your first free consultation with a nurse at either of the Affordable IVF clinics may be done via Skype, as long as you have access to a computer, the internet and a camera. This is particularly useful if you live a distance away from either clinic.After Your First Appointment
After your initial appointment, if you would like to progress with treatment, the following will need to be done:- You will need a Referral form from your GP or Specialist (one for you and one for your partner) to one of Affordable IVF’s Specialists stating that you require IVF treatment. You must have a referral before you can attend this appointment.
- Phone Affordable IVF and pay the non-refundable registration fee.
- Book a follow up appointment with an Affordable IVF nurse who will advise you on the screening tests that need to be completed and how these can be arranged. Appointment time is approximately 45 minutes.
- Once you and your partner have had all the screening tests and the results of the tests reported, a nurse will arrange for you to attend an appointment with a fertility specialist to discuss your test results and plan your treatment. Both partners must attend this first consultation with the specialist. At this appointment (approximately 30 minutes), the specialist will determine if your situation is suitable for the Affordable IVF treatment model.
